Procedural Steps for Key worker and Client

Need for structured day programme identified in care plan
ensure eligibility criteria are met
Identify activities / interventions available on DAAT website
Individual SDP plan agreed including discharge plan
Complete referral form if funding required and send to DAAT OR refer direct to activities / interventions at SDP centre
DAAT agrees funding and enrols client on activities
referrer records intervention on NDTMS
Weekly Monitoring by Key worker
Review every 6 weeks sent to DAAT
Final review sent to DAAT at end of programme. On-going needs identified and action plan agreed by keyworker / DAAT
